The Nigerian Safety Investigation Bureau (NSIB) says it has commenced an investigation into the incident involving a Fly Bird HS 125 aircraft with registration number 5N-KAL.
In a statement, the Bureau says, the aircraft, operating a flight from Nnamdi Azikiwe International Airport, Abuja, to Kotoka International Airport, Accra, experienced significant technical issues during its journey on December 6, 2024.
The Bureau emphasized that the investigation is focused on uncovering the root causes of the dual-engine failure to prevent future incidents and ensure the continued safety of aviation operations.
Preliminary information shows that the incident occurred as the aircraft, carrying three crew members and one passenger, was climbing through flight level 240 en route to flight level 280.
“At approximately 17:54 UTC, the crew reported an issue with the No. 2 engine while positioned about 25 nautical miles southwest of the VOR station VONUK”
The crew promptly requested a diversion back to Abuja, which was approved, and the aircraft began descending to flight level 190.
During the descent, NSIB notes that, the situation escalated as the No. 2 engine failed at flight level 230. Despite the setback, the crew continued their diversion to Abuja.
The situation became critical when the No. 1 engine also failed at around 5,000 feet while approaching Nnamdi Azikiwe International Airport.
Declaring a Mayday, the crew managed to execute a safe landing at Abuja at 18:16 UTC and all four individuals on board escaped unharmed.
According to the bureau, it has since deployed its investigative team, which is currently analyzing flight data, conducting interviews, and reviewing the aircraft’s maintenance records.
